Formula used
Unlike the sun sign, the moon sign needs real astronomy: the Moon moves about 13° a day, changing sign every ~2.5 days. This uses a simplified lunar-position model (with the main perturbations) to estimate the Moon's ecliptic longitude, then maps it to one of the twelve 30° signs.
Example calculation
For 15 June 1995, 12:00, the model computes the Moon's longitude and lands it in a sign. Change the time by several hours near a boundary and the sign can flip.
That time-sensitivity is exactly why moon signs need birth time, while sun signs don't.
Why the moon sign needs your birth time
